FAIRFIELD MEMORIAL HOSPITAL: FMH Opening Horizon Healthcare Grayville in Fall

Fairfield Memorial Hospital issued the following announcement on July 26.

Fairfield Memorial Hospital announced the expansion of Horizon Healthcare, a primary care practice, into the community of Grayville.  The office will officially open on September 24, 2018.

“Living in the Healthcare spectrum, we have decided that now is the right time to expand our services in other communities within our primary services area. This will give greater access and help provide easier appointment schedules for the Grayville community. Grayville is an amazing community that cares deeply about the public and has made it a mission to better equip the community with healthcare options. We are extremely grateful and dedicated to serve this community and make it our mission to give the community the services they deserve,”stated Katherine Bunting-Williams, Ph.D., Chief Executive Officer of Fairfield Memorial Hospital.    

The team that has been carefully comprised to care for those who make this medical home will include:

Christopher Ballard, MD, DABFP

Dr. Ballard, who is a native of Mount Carmel and has been with Fairfield Memorial Hospital for over 10 years, will serve as the Medical Director for the Grayville Medical Clinic and will also be seeing patients on a part time basis in the Grayville Clinic, as well as, continuing to see patients in the Fairfield office.

Sherry Funkhouser, PA

Sherry will be the Physician Assistant at the new Grayville Horizon Clinic. Sherry studied at Eastern Illinois University studying in Athletic Training and Health Education. Sherry’s goals are to work in a family practice setting and incorporate medical and education into her work force.

Kayla Bell, PA

Kayla Bell, PA-C, is a physician assistant who sees patients of all ages. She will work in the Grayville office on a part time basis. She graduated from the Physician Assistant Program at Southern Illinois University in Carbondale in 2004 with Cum Laude honors. PA Bell joined Horizon Healthcare in 2011 with years of clinical practice with experience in pediatrics, orthopedic, and emergency departments. Kayla Bell, PA-C, grew up in Grayville, IL, and is very familiar with the surrounding community and Fairfield Memorial Hospital. PA Bell resides in Albion with her husband, Joshua Bell, and children Rosen and Holland.

Misty Stone, RN

Misty will be serving as the RN in the Grayville Medical Clinic. Misty has been a nurse for over 15 years and is ACLS and PALS certified. She will be assisting patients with medication questions and refills, referrals, and nurse visit injections for allergies, vaccinations and more.

Marisa Wood

Marissa is also a native of Grayville who will be serving as the Registration Clerk/Phlebotomist in the Clinic. By having a Phlebotomist on staff, patients will be able to have lab work drawn in the Grayville Clinic without having to travel to Fairfield or other locations for their lab work. This service will be open to anyone with a physician order for lab work, not just patients being seen in the Grayville Medical Clinic.

Hollie Barrett, RN, BSN, Director of Physician Practice

Hollie Barrett, RN, BSN, has worked at Fairfield Memorial Hospital for seven years.  She has been part of the Horizon Healthcare team since 2011 and in management at Horizon Healthcare since 2013.  With nearly eight years of nursing experience and five years nurse management experience, she now serves as Director of Physician Practice.  As Director, she leads and directs the management of business and nursing at the Horizon Healthcare. She works to collaborate with the medical providers in order to maintain an efficient, welcoming atmosphere for patients to experience and be delivered the best medical care possible. Hollie is also experienced with the Patient Centered Medical Home (PCMH) program, and led the clinic to achieve PCMH recognition by the National Committee of Quality Assurance (NOQA) in February 2018. This recognition program emphasizes a PCMH program within Horizon Healthcare to achieve site recognition, which puts patients at the forefront of care.  Her office will remain at Horizon Healthcare’s main campus in Fairfield, but will be working closely with the team in Grayville to ensure a smooth opening and a seamless transition.
