Horizon Healthcare's NextGen Patient Portal is undergoing an upgrade this weekend. As a result of this upgrade, Horizon Healthcare patients who currently have the Patient Portal will be sent a link to re-register for this upgraded portal. If you are a Horizon Healthcare patient and you would like to get registered for the on-line Patient Portal, please call 618-842-4617 during regular business hours and our staff will be happy to assist you with this process!
